1. Model information
The model's name, phone, email and session date are those entered on the booking form.
2. Permission
- I understand that JXN Photography may photograph or record my likeness during the session.
- I understand that identifiable photographs may be used only for the promotional purposes I authorize below.
3. Promotional use — select each that applies
You may select more than one:
- Portfolio / Website — portfolio, website, and sample galleries.
- Social Media — JXN Photography social media accounts.
- Advertising / Paid Marketing — advertisements and paid marketing.
- Competitions / Publications — photography competitions and publications.
- Other Promotional Use — other lawful promotion related to JXN Photography.
☐ Or opt out: I do NOT authorize JXN Photography to use photographs in which I am identifiable for portfolio, website, social media, advertising, marketing, competitions, publications, or other public promotional purposes.
Selecting the opt-out does not cancel my photography session or change the agreed session fee.
4. No additional compensation
Unless otherwise agreed in writing, I will not receive additional payment for authorized use of the photographs.
5. Copyright
- JXN Photography retains copyright ownership of the photographs. This Release does not transfer copyright ownership to me.
- My personal use of the photographs is governed by the separate JXN Photography Photography Services Agreement.
6. Privacy & tagging
- This Release does not authorize publication of my private contact information, including my phone number, email address, or home address.
- Tagging or identifying me by name on social media requires my separate consent (a separate checkbox on the booking form).

- On request, vehicle license plates will be blurred or hidden in any promotional use.
7. Minors
If the person photographed is under 18, the person signing represents that they are the parent or legal guardian and have authority to provide this permission. The minor's name and the signer's relationship to the minor are recorded on the booking form. A parent or guardian signature is required when the photographed person is under 18.
8. Revocation & previously published material
- Any future request to revoke promotional permission must be made in writing.
- Revocation applies to future promotional use to the extent reasonably practicable and permitted by law, but does not automatically require removal of photographs already published, printed, distributed, reposted, or incorporated into completed materials.
- The Photographer will make reasonable efforts to stop new promotional use after receiving a written revocation.
9. Acknowledgment & signatures
- I confirm that I have read and understood this Release, have had an opportunity to ask questions, and have selected my promotional-use preferences.
- Electronic signatures and copies may be treated as valid to the extent permitted by applicable law. Typing my full name on the booking form is my signature; the date, time and version are recorded.
- This Release is governed by the laws of the State of Kansas.
